MEDICATION
If you a student needs to take prescribed medication on campus, please have your child's physician complete the form below and submit it to the Health Technician immediately.
Medications are as follows, but not limited to:
- Oral Medication
- Inhaler
- Insulin
STUDENT HEALTH HISTORY
A permanent health history is provided to the health technician every year during our Orientation process.
If your child's health history changes or you forgot to add important health information, please complete the form below and submit it directly to our health technician immediately.
