LUSD CURRENT STUDENT ENROLLMENT

 

Current LUSD students who attended LUSD elementary schools and are starting 7th Grade, they are automatically transferred to Hosler Middle School if they are within our boundaries. To find out our boundaries will click on School Locator on the right.

 

It is important that you completed the Data Confirmation on the Aeries Parent Portal prior to starting school or orientation.

INTRA-DISTRICT PERMIT PROCEDURES

 

STEP 1: Obtain intra-district permit from Student Services Dept. at the district office.

STEP 2: Parent must drop student from current school.

STEP 3: Bring intra-district permit to HMS Attendance Office.

STEP 4: Parent must complete all Aeries Data Confirmation and appropriate paperwork provided by Attendance Office staff.

STEP 5: Administration will assign student classes.

 

Once the classes are assigned, student will begin school.

 

NEW STUDENT ENROLLMENT

 

New students are registered in the Attendance Office located on campus from 7:30 – 3:00 P.M.  Parents must complete the enrollment application, registration packet, provide a Legal Birth Certificate, Custody paperwork or proof of guardianship (if applicable) Immunization records, previous school records (grades, attendance, discipline) and proof of residency (two forms of proof). If you reside outside of our District or outside of Hosler boundaries, you will need to obtain a Permit from the District Office; before you can obtain an enrollment packet and or enroll at our School.

 

It is important that packet(s) be completed BEFORE attending registration appointment! During Registration appointment, parent/guardian must complete the Aeries Parent Portal Data Confirmation section.

    • Student Emergency Card with Policy Signature on reverse side.

    • Report of Health Examination (English and Spanish)
    • Oral Health Assessment Letter (English and Spanish)
    • Oral Health Assessment Form (English and Spanish)

INTER-DISTRICT PERMIT PROCEDURES

 

STEP 1: Obtain inter-district permit from resident’s current district.

STEP 2: Parent must present said permit to LUSD Student Services Department for a LUSD inter-district permit.

STEP 3: Bring inter-district permit to HMS Attendance Office.

STEP 4: Parent must complete all appropriate paperwork provided by Attendance Office staff, and complete Data Confirmation Section in the Aeries Parent Portal.

STEP 5: Administration will assign student classes.

 

Once the classes are assigned, student will begin school.
